By participating in the Phoenix Ladies Fitness Center, I do so at my own risk.
I assume all risk of injury, illness, damage or loss to me or my property that might result, including without limitation, any loss or theft of personal property.
I consent to medical treatment in the event of injury, accident and/or illness.
I agree on behalf of myself (and my personal representatives, heirs, executors, administrators, agents and assigns) to release and discharge the organizers, its principals, its officers & directors, its employees, all sponsors and their representatives and employees from any and all claims or causes of action (known or unknown) arising out of their negligence.
I am waiving any right that I may have to bring legal action to assert a claim against the Phoenix Ladies Fitness Center and its officers and employees.
I agree to comply with all rules and policies imposed by the Phoenix Ladies Fitness Center upon my visit and/or before/after purchasing any service provided by Phoenix gym and rock climbing.
I agree to conduct myself in a controlled and reasonable manner at all times.
I agree to refrain from using any equipment in a manner inconsistent with its intended design and purpose.
I understand and acknowledge that the use of exercise equipment, group classes and The Climbing Wall involves risk of serious injury, including permanent disability and death.
I understand and agree that my use of the facilities and equipment is only to be undertaken on my own personal time, and that my use of the facilities and equipment is not within the course or scope of my employment.
I hereby warrant that to the best of my knowledge, I am in good health, and I assume all responsibility for my health.
I acknowledge that Phoenix Ladies Fitness Center is not responsible for stolen or lost items left unattended.
I am aware that the reservations for the Group Training Classes may be cancelled without penalty by 6 hours before the start of the reservation.
I acknowledge that early Group Class Cancellations are accepted by phone, email or online via KSAPHOENIX App. I am aware that if the group class cancellation occurs after the 6 hour deadline x3 in one month, I may be suspended from booking my reservations online for a period of one week.
In the case of Personal Training, Fitness Consultation Appointment, Wall Access or Drop In booking services I am aware that I must credit my account to reserve my consultation, appointment, wall access or group class no less than a minimum of 6 hours prior the desired time and in the case of not showing up or late cancelling my appointment, consultation, or group class - the consultation, drop in and/or appointment fees will be deducted from my account.
I acknowledge that in the case that I misplace or lose my complimentary access band offered free of charge at the beginning of my membership or package, that I must cover the 55 SAR fee to replace it the first time, 105 SAR the second time and 155 SAR the third time.
I understand that once I purchase a Personal Training Package, a Membership or Class Package that I am given a one month chance to activate my purchased service and if one month passes without activation, the purchased service will be expired without future use.
I understand that Group Classes on schedule and its instructors are subject to change/cancel suddenly and without prior notice.
I understand that upon participating in The Rock Climbing Wall, I do so at my own risk and agree to follow The Rock Climbing Wall’s rules and regulations set by Phoenix Ladies Fitness Center.
I understand that Phoenix Ladies Fitness Center and its staff reserve the right to refuse service to any member/client for improper conduct.
I understand that inappropriate conduct or language during my visit inside the center or by a provided service will result in immediate termination of the service.
I understand that Phoenix Ladies Fitness Center reserves the right to define inappropriate conduct.
I acknowledge that occasional changes can be made and the terms can be adjusted.
I am aware that all of the Phoenix Ladies Fitness Center are VAT inclusive as implemented by the Saudi General Authority of Zakat and Tax.
I am aware that there is a strict no refund/no exchange policy on all Phoenix Ladies Fitness Center services without exceptions.
I understand that the minimum age requirement is 16+ as regulated by the Ministry of Sports and Phoenix Ladies Fitness Center reserves the right to ask for proof of age through official governmental identification.
I understand that it is prohibited to record and take photos inside Phoenix Ladies Fitness Center in respect to the privacy of its visitors/members/employees and property.
I understand that Phoenix Ladies Fitness Center follows the Protocols, Rules and Regulations set forth by all and any governmental sector including their right to request personal data (full name, email, phone number and identification.)
By becoming a client at Phoenix Ladies Fitness Center, I agree that it may be subject to revisions and I must check updates regularly.
I ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS CONTRACT, WAIVER AND RELEASE AND PROMISE/AGREE TO FOLLOW EACH POLICY. I FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY.

أقر بممارسة الرياضة والأنشطة البدنية في مركز فينكس لرياضة السيدات على مسؤوليتي الخاصة وإدارة المركز لا تتحمل مسؤولية أي إصابة تحدث للعميل
ألتزم بإجراء فحوصات طبية للتأكد من سلامتي الصحية وخلوي من أي إصابات أو أمراض تقيدني من ممارسة الرياضة وألتزم بتقديم تقرير طبي بذلك للمركز حفاظا على سلامتي أثناء ممارستي للنشاطات الرياضية
أوافق عن نفسي والنيابة عن (الممثلين الشخصيين التابعين لي، والورثة، والمنفذين، والمسؤولين الإداريين، والوكلاء) بالتنازل وإطلاق سراح المنظمة، ورؤسائهم، مدراءهم، وموظفيهم، وممثليهم من أي ادعاءات أو تصرف ناتج عن إهمالهم
أقر بالتنازل عن أي حق لرفع دعوى قانونية ضد مركز فينكس لرياضة للسيدات ومسؤوليه وموظفيه
أقر بالالتزام بقوانين وسياسات مركز فينكس لرياضة السيدات خلال فترة اشتراكي/زيارتي للمركز
أوافق على أن أتصرف بشكل عقلاني ومنضبط في جميع الأوقات
أقر بالمحافظة على كافة الأجهزة وممتلكات المنشأة من أوزان ومستلزمات رياضية واستخدامها بعناية وأقر بأني مسؤولة عن أي تلف ناتج عن سوء استخدام
أقر بالمخاطر التي قد تترتب من ممارسة الأنشطة الرياضية والتي تتضمن الحصص التدريبة، استخدام الأجهزة والمعدات الرياضية، الأثقال، وجدار التسلق
أتعهد باستخدام مرافق المركز ومعداته الرياضية في وقتي الخاص وأن استخدامي للمرافق والمعدات ليس من ضمن سياق أو نطاق عملي
أقر بأني بصحة جيدة ولا أعاني من أي أمراض صحية تمنعني من ممارسة أي نشاط بدني
إدارة مركز فينكس لرياضة السيدات لا تتحمل أي مسؤولية حيال المفقودات الشخصية ويرجى وضع الحقائب في الأماكن المخصصة لها
لدى المشتركة الحق في الغاء حجز الحصص التدريبية قبل 6 ساعات من موعد الحصة
سياسة الإلغاء المبكر للحصص التدريبية مسموحة من خلال المكالمات الهاتفية، التواصل عن طريق البريد الالكتروني أو عن طريق تطبيق KSAPHOENIX كما يحق لمركز فينكس لرياضة السيدات بإيقاف حسابي مؤقتا (لمدة أسبوع واحد) إذا تكرر الغائي للحصص التدريبية قبل موعدها ب 6 ساعات في الشهر الواحد، ثلاث مرات.
يحق لمركز فينكس لرياضة السيدات المطالبة بالدفع مقدما (6 ساعات على الأقل) لجميع المواعيد بما في ذلك من حصص تدريبية لليوم الواحد، مواعيد تدريب شخصي ومواعيد جدار التسلق ليتم حجز وتأكيد الموعد وفي حال عدم الحضور يعتبر الموعد لاغي
في حال فقدان اسورة الدخول التي تعطى لكل مشتركة عند التسجيل، يجب دفع رسوم بملغ 55 ريال في المرة الأولى، ومبلغ 110 ريال في حال فقدانها للمرة الثانية و165 ريال في حال فقدانها للمرة الثالثة
لدى كل عميلة مهلة شهر كامل من تاريخ الدفع لتفعيل اشتراكها سواء كان اشتراك شهري، باقة زيارات أو باقة تدريب شخصي وفي حال انتهاء مهلة تفعيل الخدمة يعتبر الاشتراك/الباقة غير قابلة للتفعيل
لمركز فينكس لرياضة السيدات الحق في الغاء أي حصة رياضية أو موعد تدريب شخصي لأي ظرف طارئ
اتعهد باتباع قوانين جدار التسلق وفق لائحة مركز فينكس لرياضة السيدات وأن ممارستي للنشاط الرياضي على مسؤوليتي الخاصة وأقر بمدى خطورة النشاط
أقر بأن لمركز فينكس لرياضة السيدات وموظفيه الحق في رفض تقديم أي خدمة نتيجة صدور سلوك غير لائق من العميلة
أقر بأن التعدي اللفظي على أحد منسوبي مركز فينكس لرياضة السيدات من موظفين، مشتركين أو زوار قد يؤدي إلى إيقاف أي خدمة مقدمة من غير مقابل
أقر بأن لمركز فينكس لرياضة السيدات الحق في تحديد السلوك الغير منضبط أو المخالف للآداب العامة
يحق لإدارة مركز فينكس لرياضة السيدات التعديل على السياسات في أي وقت
يطبق مركز فينكس لرياضة السيدات ضريبة القيمة المضافة على جميع الخدمات والسلع التي يتم بيعها وفق ضوابط هيئة الزكاة والضريبة والجمارك
أقر بأن مركز فينكس لرياضة السيدات الحق في رفض استرجاع قيمة أي خدمة أو استبدالها
ألا يقل عمر المشتركة عن 16 عاما وذلك وفقا للائحة الأساسية للأندية الرياضية من وزارة الرياضة، وللمركز حق في المطالبة بإثبات عمر المشتركة
بالمشاركة في أنشطة مركز فينكس لرياضة السيدات، أتعهد بمراجعة تحديثات السياسة باستمرار
يمنع التصوير داخل المركز حفاظاً على خصوصيته وخصوصية مشتركاته وزواره وموظفيه
يحق لمركز فينكس لرياضة السيدات المطالبة ببيانات المشتركة كاملةً من اسم كامل، رقم تواصل وبريد الالكتروني وفقا للائحة الأساسية للأندية الرياضية من وزارة الرياضة
أقر بفهم العقد وأتعهد بالموافقة والالتزام على كل ما ورد فيه وعلى ذلك جرى التنويه
